Attested by a gazetted officer; or a practicing Chartered Accountant or
Advocate (or)

Attested by
the manager of a bank where the Unit holder maintains an account, specifying
the manager's bank identification number (or)

Notarized by a Notary Public. (Note: A power of attorney is always to be
notarized by a notary public.)

In case the Power of Attorney is supplied by the Unit holder, it may be
accepted in case it is substantially in accordance with the format provided.

All documents are to be executed on plain paper. Wherever stated so
specifically, documents are to be executed on non-judicial stamp paper of the
stated denomination. After completion, the required document should be
submitted to any of our Points
of Contact for resolution.
Additional
Purchase/Redemption/Switch Request
If the holder wishes to purchase additional units of the fund
he/she has already invested in or he/she wants to redeem the units they hold or
would like to switch funds from one scheme to another,

Change of
address
If the unit holder wishes to change his/her address in our
records, please click

amount was Rs. 50,000 or more, the unit holder will have to get the address
changed with CVL (click
here to download the form in case of individuals and here in case of
non-individuals) and send us the new KYC letter. Know more about

Change of Bank
A/c or mandate
The bank account details with us are used to credit dividends
and redemption amounts. If the unit holder would like to change the bank
account details, click here to download the form. The request
will have to be accompanied with a cancelled Cheque leaf as a proof of the
holder’s bank account.

Addition of
Joint Holder
If unit holder/s desires to add a second or third holder, then
they need to inform us through a letter affixing the signatures of the
second/third holder along with the PAN and its proof.

Error in the
name of the unit holder
If the unit holder's name has been erroneously recorded due to
an error by the unit holder, then to rectify the same the unit holder needs to
furnish certain documents to the Fund. If the error has occurred on the part of
the Mutual Fund or Registrar the unit holder need not furnish the documents

Unit holders, who ask for fresh dividend/redemption warrant,

Minority Status of Unit Holder

Change of name
If the change in name is requested by the unit holder the
documents mentioned below have to be furnished.

The entry in the Register of Firms relating to change of name (in case of a
registered partnership firm)

name (in case of an unregistered partnership firm).

In case of change of name of a Unit holder due to marriage, the Unit holder

Appointment of
Guardian
If the guardian is a person appointed by the court or through
legal documents the guardian needs to furnish certain documents to the fund.
Requirements:

Letter from the Guardian declaring that he/she is the guardian of the minor and
stating his/her relationship to the minor along with court order/decree, if any
-
Certified copy of proof of age of the minor (e.g. birth
certificate/passport/school leaving certificate etc.)

nominee
If the Unit holder/s wishes to add or change the nominee/s to
his Units, he/she needs to submit a letter requesting the same.

Units

Single holder
As per the offer document, nomination facility is provided for
